Canada (6 in CONCACAF in the Elo Ratings) fail to reach the first pot.
Haiti (11) and Guatemala (12) fail to reach the second pot.
Barbados (15) fail to reach the third pot.
Aruba (29) and Bahamas (30) fail to reach the sixth pot.

Special mention to Bermuda (19) and St. Lucia (23). Based on the Elo Ratings both would be in the fourth pot, but their probable FIFA Ranking puts them in the sixth pot.

Overachievers

Panama (7) in the first pot.
Antigua and Barbuda (14) and Suriname (15) in the second pot.
Dominica (22) in the third pot.
British Virgin Islands (31) and Turks and Caicos Islands (32) in the sixth pot.
